International Application: Step 3

Step 1:

Complete and return the Enrollment Agreement form, Emergency Card, and the required fees.

Step 2:

Upon receipt of these forms and payment, an appointment will be made for the parent and student to attend an orientation meeting and select classes.

Step 3:

Sign the I-20 issued to you by CCS. Your signature on the I-20 commits you to the terms outlined on page 2 of the I-20.

Step 4:

Scan and Email to the International Coordinator, Kim Blasco at kblasco@cs-w.org

NOTE: Upon arrival into the United States, submit a medical form.
